Our practice CDM nurse Madhu is also an accredited Diabetes Educator, and is available to provide personalised education and practical support to help you manage diabetes effectively and reduce the risk of long-term complications.

 

Working with a Diabetes Educator can help you better understand your blood glucose levels, medications and insulin, nutrition, exercise and lifestyle factors. Education is tailored to your goals and stage of life, empowering you to make informed decisions, improve control and maintain independence. Whether you’re newly diagnosed or looking to optimise your management, diabetes education is a valuable investment in your long-term health.

 

Madhu bulk bills patients with a GP Chronic Care Management Plan (GPCCMP), which can be arranged via a bulk billed consultation with your regular GP. A GPCCMP offers patients a Medicare rebate on up to 5 allied health consultations.

 

Alternatively, private health fund rebates apply if you are not eligible for a GPCCMP (check with your fund).
